Please fill out the form below and submit it for approval with all required dimensions and pictures. Dead trees that are endangering your property need only pictures of the trees and this request and the approval of any board member. All other additions/changes requires approval by the Committee.
By submitting this approval request form, I understand that the Committee has 30 days from the day it receives his request to act on it. Either by approval, denial or a request made to me for modifications. If the Committee fails to act within 30 days from receipt of my request, then the request shall be considered approved.
I understand that an approval by the Committee DOES NOT IN ANY WAY supersede nor override any required permits, legal injunctions, or any other required documentation or laws required or mandated by Federal, State and Local Governments. I understand that it is my responsibility to acquire all necessary building and construction permits that may be required by the state, county and/or city. I understand all permits must be supplied to the Committee PRIOR TO THE START OF ANY WORK. I understand that by approving this request, the Committee, the Board of Directors or A Quiet Place in the Woods HOA, Inc. is not to be held responsible for anything that could happen as a result of construction and./or use of the requested change/addition that was approved. I assume all responsibility for the construction and use of this change/addition. If any changes are made to the property which are not approved by The A Quiet Place in the Woods Architectural Committee in writing, I understand all fines must be paid.
(Please include accurate dimensions on any additions, such as porches, patios, decks, parking pads, etc. Also include a description of the material to be used, such as concrete, treated lumber, vinyl siding,, etc.)
